At Peace, your care team will consist of you, your family and close friends, your own physician and/or a hospice physician, hospice nurse and hospice aide/homemaker, social worker, spiritual and bereavement counselors, as well as hospice volunteers. Others who may be involved in your care include but are not limited to: dietician, physical, speech and occupational therapists, music therapist and pharmacist.
