Home Health Care offers personal assistance in the privacy and comfort of your own home. We provide Medicare-certified home health care and private duty services focused on individual well-being. Skillful care coordinators extend the work of our Ministries, matching your needs with the right resources. Holy Family Hospice brings peace and comfort to patients and families when life is measured in months, weeks or days. Our goal as a hospice is, and always will be, to remain grounded in our mission and to never lose sight of the ever-changing, individualized needs of our patients as they enter this next phase of life's journey. Hospice is not a place, but a philosophy of care focused on the individuals comfort and wishes. Those who use hospice care are given the opportunity to determine where they will receive services and how much or how little support they need with the help of their specialized hospice team. Holy Family Hospice patients may choose to stay at home, in an extended care facility or in our residential facility for hospice services. Regardless of where they live, Holy Family Hospice offers a holistic approach, using traditional and complimentary comfort therapies, that help patients and families cope with the complexity of the end-of-life process.
